I agree to the children within my group being filmed or photographed during the activity, with the possibility that these photographs/media recordings may be used for publications or marketing publicity. Hartlepool United FC will take all steps to ensure these images are used solely for the purposes for which they are intended. Note: if consent is not given, Hartlepool United FC will not use any images.

I will notify Hartlepool United FC should any individuals within the group not provide consent.

I release Hartlepool United from any liability that may arise due to the publication of such information.

Full payment of £300 will be taken on receipt of the signed paperwork. We reserve the right to charge a 50% cancellation charge if you release your booking between six and three weeks from the event and thereafter will charge a full cancellation charge unless we manage to re-sell the opportunity.

In the event of an abandonment your booking will be transferred to the next appropriate event.
